Lissy’s Legacy of Love – Easter Basket Program

Lissy’s Legacy of Love is dedicated to supporting children affected by addiction. This includes children who:

• Have a parent incarcerated on drug-related charges

• Have been removed from the home due to parental drug use

• Have lost a parent to an overdose

• Are living with relatives or in foster care due to substance abuse

Application & Pick-Up Information

Please complete all required information on the application. Once your application is approved, we will mail you a letter with the Easter basket pick-up date, time, and location.

Due to limited resources, we will only offer one designated day and time for basket pick-up. This program is very time-consuming, and we kindly ask that if you sign up, you commit to picking up your child’s basket.

Please note:
If you are a no-show for any holiday pick-up without prior communication, we will no longer be able to assist your family in future holiday programs. Unfortunately, we have had too many items left unclaimed, which has led us to implement this policy.

Required Documentation

Within 24 hours of submitting the application, you must email us the following documents for each child:

• A copy of the child’s birth certificate

• Custody or guardianship paperwork

⚠️ Applications will not be processed without this documentation.

If you have already submitted these documents for a previous holiday, you do not need to resend them unless your situation has changed.

Important Notes

• A separate application is required for each child

• A new application is required for each holiday, as information such as sizes and interests may change
